To make this happen is very simple. startx, open a xterm, then iconify it. It doesnt show up in the icon menu or alt+tab. The funny thing is, it is not 100% gone, just not retrievable. If I would open another xterm, it would be xterm #2 in the title bar. Also if I would exit back to the console, the missing xterm would flash for a second as openbox was exiting.   Thats just a for sure way to reproduce this. It seems to happen randomly but the first xterm will always do it.
