That really isn't very useful. I couldn't find any other bug reports (or I wouldn't have filed this one) and the mailing lists don't seem to be searchable so could you please tell me who's problem it is (and maybe what the problem is) so I can go and hastle them instead?

You have either a) an unaccelerated mesa libgl (as in, not related to your working dri) or b) some other driver screw up.
Get an nvidia card, read the lgfaq: http://www.icculus.org/lgfaq/
If you're running at 16bpp, it may not be your fault. It could be this bug:
https://bugzilla.icculus.org/show_bug.cgi?id=2796
which also causes a GLimp_Init() failure on my machine when unfixed.
If you can, try that patch and see if it fixes your problem?
Setting a QA contact on all ioquake3 bugs, even resolved ones. Sorry if you get a flood of email from this, it should only happen once. Apologies for the incovenience.
