Mac OS X, both the CMD and ALT keys are recognized as "ALT"

This is potentially something that would need SDL prodded and in any case might be difficult to get a sensible mapping working that applies in a sane way to every OS. If you come up with some idea about how to resolve this or a patch, reopen.
